Release of Liability

I do require that a Release of Liability is signed by both parties.  

CJMECH Release of Liability

Please read carefully before signing.

In consideration of the services provided by Carter Hausladen owner of CJMECH, release, waive, discharge, and covenant not to sue CJMECH, agents, and representatives, from any and all liability, claims, or losses of any kind or nature, whether known or unknown, arising out of, connected with, or in any way related to the services provided by CJMECH.

I understand and acknowledge that the services provided by CJMECH may involve certain risks, including but not limited to:

• Personal injury

• Other unforeseen risks

I voluntarily assume all risks associated with the services provided by CJMECH and agree to hold harmless Carter Hausladen owner of CJMECH from any and all liability arising from such risks.

I have carefully read this Release of Liability and fully understand its terms. I sign this Release of Liability voluntarily and without any inducement or assurance other than those expressly stated herein.
